A minor must be accompanied by a parent. The parent must present a valid photo ID and provide proof of relationship to the minor.
Examples: driver’s license, passport, learner’s permit, school ID, report card with the parent and child’s names, health insurance card with matching names and/or numbers, or a yearbook.
If the parent and minor’s last names are different, you will need to provide the minor’s birth certificate in addition to the parent’s photo ID.
